For patients who have spent years searching for a solution in vain, the scientific community is increasingly considering innovative, psychedelic-based interventions.<\/p>\n<p data-path-to-node=\"4\">Over the last decade, the scientific world has witnessed a so-called <b data-path-to-node=\"4\" data-index-in-node=\"69\">&#8220;Psychedelic Renaissance.&#8221;<\/b> This is a period where substances that were taboo for decades have returned to leading laboratories and clinical trials\u2014this time accompanied by strict medical oversight and cutting-edge technology. Beyond temporary symptom relief, this approach aims for fundamental changes in functional brain dynamics and the restoration of neuroplasticity.<\/p>\n<p data-path-to-node=\"5\">In this article, <b data-path-to-node=\"5\" data-index-in-node=\"17\">Davit Sagharadze<\/b>, the only internationally certified psychedelic therapist in Georgia, shares his 35 years of clinical experience and the knowledge he gained in Canada. Sagharadze explains in detail the principles of <b data-path-to-node=\"5\" data-index-in-node=\"234\">Ketamine-Assisted Psychotherapy (KAP)<\/b>, the neurobiological mechanisms of the drug\u2019s action, and the unique opportunities this approach creates in managing treatment-resistant depression.<\/p>\n<p data-path-to-node=\"6\"><em><strong><span style=\"color: #0000ff;\">Mr. Davit, what is Treatment-Resistant Depression?<\/span><\/strong><\/em><\/p>\n<p data-path-to-node=\"7\"><b data-path-to-node=\"7\" data-index-in-node=\"0\">Treatment-Resistant Depression (TRD)<\/b> is a condition where a patient does not respond to at least two different antidepressant treatments administered at an adequate dose and duration. Often, this means years of various therapeutic attempts, frustration, and a significant decline in functioning. In such cases, the need for alternative approaches arises.<\/p>\n<p data-path-to-node=\"8\"><em><strong><span style=\"color: #0000ff;\">What does &#8220;Psychedelic Therapy&#8221; mean?<\/span><\/strong><\/em><\/p>\n<p data-path-to-node=\"9\">Psychedelic substances have been used for millennia in ritual and healing practices. Modern research studies how certain substances can temporarily increase neuroplasticity and facilitate the psychotherapeutic process. Classic psychedelics (LSD, DMT, Ibogaine, Mescaline) are mostly in various stages of research and are not yet widely used in clinical practice. However, the use of <b data-path-to-node=\"9\" data-index-in-node=\"383\">Psilocybin<\/b> and <b data-path-to-node=\"9\" data-index-in-node=\"398\">MDMA<\/b> for treating depression and PTSD is very close to FDA approval.<\/p>\n<p data-path-to-node=\"10\"><em><strong><span style=\"color: #0000ff;\">What is the legal alternative today?<\/span><\/strong><\/em><\/p>\n<p data-path-to-node=\"11\">Currently, one of the legally available approaches for treatment-resistant depression is <b data-path-to-node=\"11\" data-index-in-node=\"89\">Ketamine-Assisted Therapy (KAP)<\/b>. Ketamine is not a classic psychedelic. It acts on the glutamate system and can rapidly reduce depressive symptoms.<\/p>\n<p data-path-to-node=\"12\"><em><strong><span style=\"color: #0000ff;\">How does clinical ketamine therapy differ from non-medical, recreational use, and why is self-medication dangerous?<\/span><\/strong><\/em><\/p>\n<p data-path-to-node=\"13\">The difference is fundamental. Recreational use is associated with serious risks:<\/p>\n<ul data-path-to-node=\"14\">\n<li>\n<p data-path-to-node=\"14,0,0\"><b data-path-to-node=\"14,0,0\" data-index-in-node=\"0\">Psychological Risks:<\/b> Panic reactions, delirium, prolonged depersonalization (the so-called &#8220;K-hole&#8221;), and the provocation of psychosis.<\/p>\n<\/li>\n<li>\n<p data-path-to-node=\"14,1,0\"><b data-path-to-node=\"14,1,0\" data-index-in-node=\"0\">Physical Dangers:<\/b> &#8220;Street ketamine&#8221; is not pure and often contains fentanyl or benzodiazepines, posing a risk of respiratory depression.<\/p>\n<\/li>\n<li>\n<p data-path-to-node=\"14,2,0\"><b data-path-to-node=\"14,2,0\" data-index-in-node=\"0\">Organ Damage:<\/b> Long-term use in high doses causes irreversible damage to the bladder walls.<\/p>\n<\/li>\n<li>\n<p data-path-to-node=\"14,3,0\"><b data-path-to-node=\"14,3,0\" data-index-in-node=\"0\">Cognitive Decline:<\/b> Impairment of memory and executive functions during chronic use. In a clinical setting, only chemically pure pharmaceutical-grade products are used, with precise dosing and constant medical supervision.<\/p>\n<\/li>\n<\/ul>\n<p data-path-to-node=\"15\"><span style=\"color: #0000ff;\"><em><strong>How do interventions like DMT or Ketamine manage to change brain patterns that standard antidepressants couldn&#8217;t tackle?<\/strong><\/em><\/span><\/p>\n<p data-path-to-node=\"16\">Clinical trials have shown that a single 25-minute DMT session resulted in sustained improvement for six months in patients with TRD. In cases of TRD or OCD, the brain is often &#8220;locked&#8221; in repetitive negative patterns\u2014particularly in self-referential and ruminative networks. In a therapeutic regimen, Ketamine is not used daily\u2014the minimum interval between sessions is 72 hours (averaging two ketamine sessions and one integration session per week). Furthermore, a strict medical evaluation is conducted beforehand, and blood pressure, pulse, and respiration are constantly monitored during the session in the presence of an anesthesiologist and a therapist.<\/p>\n<p data-path-to-node=\"20\"><span style=\"color: #0000ff;\"><em><strong>What is the difference between Ketamine and antidepressants in terms of efficacy?<\/strong><\/em><\/span><\/p>\n<p data-path-to-node=\"21\">KAP is characterized by a faster clinical response: a reduction in symptoms is observed in <b data-path-to-node=\"21\" data-index-in-node=\"91\">70\u201385%<\/b> of cases within 24\u201372 hours (compared to 50\u201365% for antidepressants). The sustained remission rate (\u22653 months) for KAP is <b data-path-to-node=\"21\" data-index-in-node=\"220\">50\u201370%<\/b>, whereas, with the standard approach, it ranges between 30\u201340%. Importantly, without psychotherapeutic integration, the long-term effect of ketamine is significantly lower (20\u201330%).<\/p>\n<p data-path-to-node=\"22\"><em><strong><span style=\"color: #0000ff;\">How does Ketamine work on a neurobiological level?<\/span><\/strong><\/em><\/p>\n<p data-path-to-node=\"23\">Ketamine reduces pathological hyperactivity in certain brain regions and promotes <b data-path-to-node=\"23\" data-index-in-node=\"82\">synaptogenesis<\/b>\u2014the formation of new neuronal connections. This process results in a &#8220;window of opportunity&#8221; (24\u201348 hours) when the brain is more plastic and open to new patterns of thinking. Ketamine creates the biological conditions that enhance the impact of psychotherapeutic work.<\/p>\n<p data-path-to-node=\"24\"><em><strong><span style=\"color: #0000ff;\">On a subjective level, what does the patient feel during the session?<\/span><\/strong><\/em><\/p>\n<p data-path-to-node=\"25\">A session lasts 45\u201360 minutes. The patient may experience altered perception, visual imagery, emotional release, dissociation, or a feeling of weightlessness. The process takes place in a calm environment, with an eye mask, specialized music in headphones, and the constant presence of a therapist, ensuring safety and emotional comfort.<\/p>\n<p data-path-to-node=\"26\"><em><strong><span style=\"color: #0000ff;\">For which conditions is it used?<\/span><\/strong><\/em><\/p>\n<p data-path-to-node=\"27\">Ketamine-Assisted Therapy is used for treatment-resistant depression, severe depressive episodes, the need for rapid reduction of suicidal ideation, as well as PTSD, anxiety disorders, addictions, and cognitive-emotional disorders.<\/p>\n<p data-path-to-node=\"28\"><span style=\"color: #0000ff;\"><em><strong>How does KAP work in practice?<\/strong><\/em><\/span><\/p>\n<p data-path-to-node=\"29\">The process is based on three stages: <b data-path-to-node=\"29\" data-index-in-node=\"38\">Preparation \u2013 Session \u2013 Integration<\/b>. During preparation, a psychiatric evaluation is conducted. The session is held under medical supervision. The integration stage is critical, as this is when the &#8220;window&#8221; opened by the ketamine is used to consolidate new experiences.<\/p>\n<p data-path-to-node=\"30\"><em><strong><span style=\"color: #0000ff;\">What are the main contraindications?<\/span><\/strong><\/em><\/p>\n<p data-path-to-node=\"31\">Therapy is not recommended for psychotic disorders, the manic phase of bipolar disorder, uncontrolled hypertension, severe cardiovascular pathologies, active substance abuse, and pregnancy.<\/p>\n<p data-path-to-node=\"32\"><em><strong><span style=\"color: #0000ff;\">What is the outlook in Georgia?<\/span><\/strong><\/em><\/p>\n<p data-path-to-node=\"33\">Awareness in Georgia is still low.
