Yet AEDs are not evenly distributed, and road-based retrieval can be slow when devices are far away, traffic is heavy, or incidents occur in areas with limited coverage.

Researchers from France examined cardiac-arrest data from seven departments in the Île-de-France region, excluding central Paris. They compared three strategies: adding more fixed AEDs, delivering AEDs by drone, and combining the two approaches.

The hybrid strategy performed best

The model estimated that a combined network of fixed AEDs and drones could reduce access time by an average of 3.76 minutes. More than 95% of simulated cardiac-arrest cases could receive an AED within five minutes, compared with approximately 30%–40% when relying on road transport from existing fixed AED locations.

The researchers estimated that 200 drone bases and 871 fixed AED sites could provide coverage for 99.4% of the 28,349 cardiac arrests included in the model. In some locations, the estimated time saved exceeded 10 minutes.

The benefit was greatest in peripheral and congested areas, where road access or the existing distribution of AEDs was less favourable.

Under the proposed model, drones would be operated by trained personnel or automated systems under emergency-service supervision. A bystander at the scene would retrieve the delivered AED and use it while receiving guidance from emergency dispatchers.

The strategy would therefore add another link to the chain of survival; it would not replace bystander CPR, emergency medical teams, or the need for people to know how to use an AED.

Important limitations

The findings are based on a simulation, not a real-world deployment. Drone travel times were calculated using straight-line distances and did not account for weather, airspace restrictions, regulations, obstacles, battery limitations, landing conditions, or other operational challenges.

The results therefore show potential rather than proven clinical benefit. Prospective studies and real-world trials will be needed to determine whether drones can consistently deliver AEDs quickly and safely—and whether that translates into more timely defibrillation and improved survival.

The study’s main message is that emergency access depends not only on how many AEDs exist, but also on where they are located and how they are organized. In some settings, drone delivery could complement fixed AED networks and help reduce geographic inequalities in access to lifesaving treatment.

Source: European Society for Emergency Medicine (EUSEM)

Drone-delivered automated external defibrillators (AEDs) could improve access to early defibrillation after out-of-hospital cardiac arrest, particularly in rural areas and congested urban settings, according to a computer simulation presented at the European Emergency Medicine Congress.
