At the American Academy of Ophthalmology (AAO) Annual Meeting, biotechnology company Life Biosciences presented promising Phase 1 clinical trial results for ER-100, a groundbreaking therapy for the treatment of glaucoma.

The experimental therapy is based on the discovery of cellular reprogramming (reverting cells to an initial, embryonic-like state) by 2012 Nobel laureate Shinya Yamanaka. Yamanaka originally utilized four specific genes, but full reprogramming posed a risk of teratoma (tumor) formation.

David Sinclair, Professor at Harvard Medical School, along with his colleague Yuancheng Lu, modified this approach—they eliminated the fourth, cancer-associated gene and utilized only three (OCT4, SOX2, and KLF4). The goal of this novel approach is not to revert cells to an embryonic stage, but rather to “rewind” them to a point when they were healthy and fully functional.

First Results of the Clinical Trial

The therapy targets retinal ganglion cells (RGCs), which connect the eye to the brain. In glaucoma, elevated intraocular pressure damages these neural pathways, leading to irreversible vision loss.

Phase 1 enrolled 3 patients with open-angle glaucoma (OAG). Each patient received a single intravitreal injection of ER-100.

Safety: At day 56, the therapy was well tolerated by all three patients, with no serious adverse events reported.

Visual Improvement: While the primary objective of Phase 1 is to evaluate safety, two out of the three patients already demonstrated significant improvement in their visual field—regaining the ability to perceive points of light they could not detect prior to therapy.

To ensure complete control over the process, scientists engineered a unique safety mechanism. The rejuvenating genes delivered into the eye do not function autonomously; to activate them, patients take the oral antibiotic doxycycline for eight weeks, which acts as an “on-off switch.” Once doxycycline administration ceases, the cellular reprogramming process stops.

What Lies Ahead?

Based on these encouraging safety and efficacy signals, the Data Safety Monitoring Board (DSMB) recommended escalating to a three-fold higher dose (Dose Level 2).

If validated in larger-scale trials, this paradigm could transform treatment not only for optic neuropathies (including NAION), but also broadly across numerous age-related conditions. In preclinical animal studies, David Sinclair’s laboratory has already demonstrated promising cell rejuvenation results in the brain, knee, spine, and liver.

TIME

For the first time in the history of clinical trials, scientists have successfully achieved epigenetic “rejuvenation” of human visual cells and restored their function.
