One tablet to treat sleep apnea: AD109, a new drug in phase 3 research

Obstructive sleep apnea remains one of the most common yet frequently undiagnosed sleep disorders worldwide. The condition develops when the upper airway repeatedly collapses during sleep, temporarily blocking breathing. As a result, the body receives insufficient oxygen, patients experience repeated micro-awakenings, and normal sleep architecture becomes disrupted. Beyond loud snoring and excessive daytime sleepiness, the disorder significantly increases the risk of hypertension, heart attack, stroke, heart failure, and metabolic diseases.

For many years, the primary treatment for sleep apnea has been CPAP therapy, which uses continuous positive airway pressure to keep the airways open. Although this method is highly effective, many patients struggle to tolerate it. Some find the mask uncomfortable, others experience disturbed sleep, and many eventually discontinue treatment altogether. For this reason, researchers have long searched for alternatives that directly target the underlying mechanisms of the disease while being more convenient for patients.

In 2026, the American Thoracic Society (ATS) presented findings that attracted considerable attention. The focus was AD109, a once-nightly pill designed to address one of the major causes of obstructive sleep apnea — airway collapse during sleep. According to the researchers, this is the first pharmacological therapy aimed directly at the neuromuscular mechanisms of the upper airway rather than merely controlling symptoms.

AD109 combines two active compounds, aroxybutynin and atomoxetine. These substances help maintain the tone of throat and tongue muscles during sleep, reducing the likelihood of airway obstruction. In obstructive sleep apnea, relaxation of these tissues is one of the primary reasons airflow becomes blocked. The goal of the new therapy is to keep these muscles more active throughout the night, allowing uninterrupted breathing.

In a large Phase III clinical trial, patients receiving the medication experienced a significant reduction in episodes of interrupted breathing. Researchers observed improvements in oxygen saturation as well as reductions in overall disease severity. In some cases, the improvement was substantial enough to suggest near-complete disease control.

However, like most new medications, AD109 also has limitations and potential side effects. During clinical studies, some patients reported nausea, insomnia, anxiety, and urinary difficulties, leading several participants to discontinue treatment. Specialists therefore emphasize that the drug may not be the ideal option for every patient, and further work is needed to determine who is most likely to benefit from it.

In recent years, several other therapeutic approaches for sleep apnea have also emerged. One of the most promising is hypoglossal nerve stimulation — an implanted device that stimulates tongue muscles to help maintain an open airway during sleep. Studies published in 2026 suggest that this technique is both effective and relatively safe for patients who cannot tolerate CPAP therapy.

Researchers are also investigating GLP-1 medications, originally developed for diabetes and obesity treatment. Evidence suggests that weight reduction associated with these drugs may also lessen the severity of sleep apnea, since excess fatty tissue around the airway contributes to collapse and obstruction.

Despite these advances, physicians continue to stress that obstructive sleep apnea remains significantly underdiagnosed. Many people consider loud snoring merely an annoyance, when in reality it may indicate a serious medical disorder. Persistent fatigue, morning headaches, impaired concentration, and excessive daytime sleepiness are symptoms that should not be ignored.

The development of this new medication could represent a major turning point in sleep medicine. If future studies continue to confirm its effectiveness and safety, patients may eventually have a much wider range of treatment options — from masks and implanted devices to a simple pill taken once each night.
