The implementation of innovative approaches in modern diagnostic practice significantly simplifies the process of identifying endometriosis. In a study conducted by French researchers, which included 971 patients aged 18 to 43, the effectiveness of a saliva-based test, Endotest, was evaluated. To confirm the pathology, specialists utilized the analysis of specific micro-RNA (miRNA) markers present in the saliva.
This technological innovation significantly reduces the established average 7-year interval for diagnosing endometriosis. Previously, patients’ quality of life was limited by long-term chronic pain and the necessity for invasive manipulations. France began clinical testing of the test in February 2025, with costs fully reimbursed by state insurance.
High Accuracy of the ENDOmiRNA Study
A group of researchers from the company Ziwig conducted a large-scale clinical study in 17 medical centers across France. Out of 971 patients with clinical signs of endometriosis, the pathology was confirmed in 77% of cases. In the diagnostic process, the saliva test examined specific miRNA biomarkers, which function as small regulators of genes.
The results obtained are characterized by high reliability: the overall diagnostic accuracy reached 96.6%. The test revealed particular diagnostic value in those complex clinical cases where, against a background of pronounced symptoms, radiological research results were ambiguous.
In comparison, tomographic research showed an incorrect diagnosis in 27% of cases, while the saliva test allowed an error in only 4.6%. Notably, the study results were not influenced by the patient’s age or the intake of painkillers or hormonal medications.
Professor François Golfier of the Lyon-Sud Hospital presented the research data at the 2025 Paris Women’s Health Congress. He stated that the characteristics of Endotest significantly exceed established international standards.
The Endobest Study
Following the confirmation of the test’s validity, a large-scale study named Endobest began. This stage directly involves 2,500 patients, and a total of 25,000 people. The French health authority fully supported the realization of the project. The national insurance fully reimburses the cost of the test, which amounts to 839 euros. Preliminary data from the study will be published in mid-2026, which will finally establish new standards for the diagnosis of endometriosis.
Discussion surrounding the issue continues in medical circles. Some experts question the clinical significance of those small foci whose identification is not possible through visualization studies. According to Professor Golfier’s explanation, a positive test result does not automatically imply the necessity of surgical intervention. Pain syndrome is multifactorial, which is why the course of treatment also includes hormonal correction and symptomatic therapy.
France’s National Strategy
Since 2022, a national strategy for combating endometriosis has been in effect in France. Specialized medical networks have been established in every region of the country. This system ensures the effective management of the treatment process and facilitates the realization of innovative scientific projects.
