Every year, thousands of children require severe dental interventions, including general anesthesia, due to untreated tooth decay. A major new clinical trial conducted by the University of Michigan confirms that a simple and inexpensive liquid, silver diamine fluoride (SDF), can spare children from these complex procedures. Applied to the affected tooth surface with a small sponge in just a few seconds, this formulation stops caries progression without drilling, injections, or sedation.
Published in the journal JAMA Pediatrics, the Phase 3 clinical trial included 830 children under the age of 6. Results showed that applying 38% silver diamine fluoride once every 6 months arrested caries progression in primary teeth in more than half of the cases. Unlike traditional methods involving the removal of decayed tissue and filling, this new approach is significantly simpler, painless, and safer.
Although silver diamine fluoride has been used in the US since 2014 to treat tooth sensitivity, there was previously insufficient clinical evidence for Food and Drug Administration (FDA) approval as an anti-caries medication. The new study, involving researchers from the Universities of Michigan, New York, Iowa, and Indiana, provides this essential scientific framework, paving the way for broader adoption and official recognition.
The sole drawback of the method is cosmetic, as silver permanently darkens the affected area of the tooth. However, experts emphasize that its practicality is invaluable for young children, elderly individuals, people with physical or developmental disabilities, and those with severe dental anxiety. In many instances, periodic reapplication until the primary tooth naturally sheds is fully sufficient to prevent pain and infection.
