Arthritis drug significantly restores hair in patients with severe hair loss: Study

Data from a new, large-scale clinical trial published in the journal JAMA Dermatology indicates that upadacitinib, a medication widely used to treat rheumatoid arthritis, significantly promotes hair regrowth in patients with severe forms of alopecia areata.

Alopecia areata is an autoimmune disease in which the immune system mistakenly attacks healthy tissue (hair follicles), causing body hair loss and the formation of patchy bald spots. Upadacitinib belongs to the class of JAK (Janus kinase) inhibitors. It blocks specific proteins involved in the inflammatory and autoimmune processes that trigger hair loss.

Clinical Trial Design and Results

Researchers analyzed two Phase 3 international clinical trials (UP-AA1 and UP-AA2) involving a total of 1,399 participants (adolescents aged 12 to 17 and adults up to age 64). The trial enrolled only patients who had already lost at least 50% of the hair on their scalp. Participants were divided into three groups: one received a 15 mg dose, another received a 30 mg dose, and the third group received a placebo.

Results obtained after 24 weeks demonstrated:

  • With the 15 mg dose, nearly half of the patients regrew more than 80% of their lost hair, while more than one in three patients (over 35%) achieved greater than 90% hair coverage.

  • With the 30 mg dose, more than 54% of participants achieved at least 80% hair regrowth (furthermore, more than half of this group experienced significant regrowth of eyebrows and eyelashes as well).

  • By comparison, only 2% of participants in the placebo group showed 80% hair regrowth.

Notably, the first positive signs of hair growth were documented as early as week 8 of therapy. Beyond physical outcomes, patients reported notable improvements in their social and emotional well-being—a critically important factor in managing this condition.

Trial Limitations

Despite high clinical efficacy, the use of such immunosuppressive drugs requires caution. The most common adverse events recorded during the trial were acne, the common cold, throat irritation, and upper respiratory tract infections. Although serious adverse events were quite rare, the drug should strictly be prescribed under medical supervision following an individual patient risk assessment.

According to the authors, these findings are highly promising. The trial demonstrates that upadacitinib has the potential to become an important therapeutic alternative for adolescents and adults with severe alopecia—especially given that effective medical options for managing severe forms of the disease remain quite limited today.
